What do you do when someone doesn't want to help themselves?

What do you say to make them realise they are harming themselves?

It's been going on for too long now with really minimal improvement, if any... there's only so much a nice chat can do you know.

And I hope that it doesn't have to get to the point where she's in hospital and finally realises what's happening.
